Having followed the kills and drops from the leading guilds of the world, I'd like to say:

T5 Head - Lady Vashj (Serpentshrine Cavern)
T5 Shoulders - Void Reaver (The Eye)
T5 Hands - Leotheras the Blind (Serpentshrine Cavern)
T5 Chest - Kael'Thas (The Eye)
T5 Legs - Fathom-Lord Karathress (Serpentshrine Cavern)

Now I haven't seen proof yet of Kael'thas dropping the chest, but really he's the final Boss of one of the instances from which these drop, and only the chest hasn't been seen, afaik. I'm going to say the odds could be pretty good that he'll drop them.

Of course should the chest actually drop in Mount Hyjal, well...

These are for Tier 5 armor.

The current Drop Locations are as Follows

Tempest Keep Raid
Serpentshrine Cavern Raid
Battle for Mount Hyjal Raid

2 in TK Kael'Thas and another
2 in SSC Lady Vashj and Another
1 in Hyjal

Tier 6 will be dropping in Black Temple and most likely will be tokened as well with the Ashtongue Deathsworn Rep needed. Think a new Zul'Garab/Naxxramas Loot system. Only speculation at this point.

"These items drop from end-game instances, the 25 man raids. There are a total of 3 different kinds of sets, for each tier. Which I'll sum up here.

For every class Tier 4 is from 'fallen' Teir 5 is from 'Vanquished'
(i.e. leggings of the fallen hero = Tier 4 leggings, leggings of the vanquished defender = Tier 5 leggings)

Mage / Warlock / Hunter = The 'Hero' sets

Paladin / Rogue / Shaman - The 'Champion' sets

Warrior / Priest / Druid - The 'Defender' sets

In Beta you could expect the Tier 4 ('Fallen') pieces to drop in 'Serpenshrine Cavern' and 'The Eye', Records show that they could also drop from lower areas, such as Magtheridon's Lair, Gruul's Lair, and Karazhan (only a 10 man).

Tier 5 drops mainly from 'Battle for Mount Hyjal' and 'Black Temple'.

When you get one of these pieces, you go to the faction you are allied with in Shattrath (Terrokar Forest), either the Aldor or the Scyers. There you can turn in the piece to obtain the Tier 4/5 armor for your class. :

t5 drops starting in serpentshrine cavern up to the battle of mount hyjal. tier 6 will drop in the black temple.

It says
Tier 4 raid set: This armor can be obtained from Asuur (Aldor) or Arodis Sunblade (Scryer) in Shattrath City using tokens obtained from Karazhan, Gruul's Lair, and Magtheridon's Lair.
Tier 5 raid set: This armor can be obtained from Kelara (Aldor) or Veynna Dawnstar (Scryer) in Shattrath City using tokens obtained from Serpentshrine Cavern and Tempest Keep.

Is this an item that everybody could loot or only one lucky guy gets?

T4 - "Of the Fallen ?"
Karazhan (Gloves, Helm)
Gruuls Lair (Shoulders, Legs)
Magtheridon's Lair (Chest)

T5 - "Of the Vanquished ?"
Tempest Keep - The eye (Shoulders, Chest)
Serpentshrine Cavern (Legs, Gloves, Helm)

T6 - "Of The Forgotten ?"
Battle for Mount Hyjal (Gloves, Helm)
Black Temple (Shoulders, Legs, Chest)

The gloves and helm in Karazhan only drop once per raid as they are 10 mans. All 25 man raid bosses that drop teir items drop 2 tokens. Only ONE person per raid will loot a token.

There are 3 types of tokens that can drop per boss:

"Of the ? Champion" - Paladin, Shaman, Rogue
"Of the ? Defender" - Warrior, Priest, Druid
"of the ? Hero" - Warlock, Mage, Hunter

You can only use your classes' token type to buy your teir from a vendor.

The Teir 4/5 vendors are in Shattrath City. It is the Scryer/Aldor rep. The T6 vendors are at the beginning of The Battle For Mount hyjal.

I hope I cleared things up for you guys, good luck on those tokens :)

EDIT: i think bosses 25 man bosses will now drop 3 tokens, this will definatly speed up gearing up a raid. (As of patch 2.4)

just discovered that you can use your tokens to purchase Merciless Gladiators (pvp) set as well, on the Isle of Quel'Danas
