This is actually used in a quest in Dustwallow Marsh. Nat Pagle gives it to you and tells you to go swim to a shipwreck and use it on yourself.

This is used in the quest Nat's Bargain which is the 5th part of a quest chain that starts with This Old Lighthouse

Use on yourself and go to 55,62, near the broken ship.
Here will spawn a shark, defeat him and you have done!

The quest for this is called Nat's Bargain. Pre-req was another quest in Thermore that sent me to Nat Pagle. Sorry can't remember what the previous quests were called.
Lvl 35 Warrior completed solo. The Lurking Shark is lvl 37 non-elite.
There are also lvl 35-37 Muckshells in the area. Some can heal. I suggest before you put that stuff on, go over and clear a bit.

The reward is:
3700 exp
40 silver
2 Darkmoon Special Reserve

Next quest you get from Nat Pagle is "Oh, It's real", which sends you back to the docks to talk to Major Mills.

Ugh the next part is Take Down Tethyr.
There are 12 marksmen on the docks. You have to run around to 3 cannons and shoot him while the marksmen pick him off. You wont take any damage from what I saw. I just ran around to each cannon, doing about 10% each hit on the cannon on him. The problem is he does a splash knockback thing. It threw me out of range and I failed the quest with 6 marksmen still standing.
When I failed it Major Mills despawned and I waited but he didn't come back! I'll try again later or tomorrow. I hope he comes back!

You can put on the fish paste and then swim about while using stealth or prowling, thereby avoiding murkshells while waiting for the shark to attack. With cat mode and Track Humanoids, it wasn't hard to stay away from the murkshells either.
