In case unknown:

It can be worn as a necklace.
One does need it in your inventory to zone into Onyxia's lair.
One does NOT need to be wearing it to zone into Onyxia's lair.
It cannot be DE'd.
Fairly sure it can be destroyed, not 100% on that.
As said, duplicates can be picked up.

Without further ado,
Walkthrough - Drakefire Amulet:

1) Go to Kargath and speak to Warlord Goretooth in the tower. Click his conversation item repeatedly to receive Warlord Goretooth's Command. Use this item to receive the quest, Warlord's Command.

Warlord's Command
Slay Highlord Omokk, War Master Voone, and Overlord Wyrmthalak. Recover Important Blackrock Documents. Return to Warlord Goretooth in Kargath when the mission has been accomplished.

Highlord Omokk slain: 0/1
Overlord Wyrmthalak slain: 0/1

2) You will have to five-man Blackrock Spire (BRS). The documents spawn randomly at one of the three bosses above and only one per instance can be acquired. This quest may NOT be raided!

3) Upon completion of Warlord's Command, you will receive the quest, Eitrigg's Wisdom.

Seek counsel with Thrall's advisor, Eitrigg. He can be found in Thrall's chamber.

4) Speak to Eitrigg in Thrall's Chamber (speech bubble), then return to Thrall. He will give you the quest, For the Horde!

For the Horde!
Travel to Blackrock Spire and slay Warchief Rend Blackhand. Take his head and return to Orgrimmar.

4) You will have to attend a raid of Upper Blackrock Spire (uBRS) to get the head of Rend Blackhand. Do so, and once again return to Thrall in Orgrimmar. All the occupants of Orgrimmar will receive a buff upon turn in.

5) Thrall will tell you to seek out Rexxar, who roams from north Feralas, through Desolace, and into southern Stonetalon Mountains. For maps of his route, see https://www.wowhead.com/tbc/search?q=rexxar . Find Rexxar.

6) Rexxar will give you the quest, The Testament of Rexxar. You are to deliver Rexxar's Testament to deliver to Myranda the Hag in Western Plaguelands.

7) Find Myranda the Hag near Uther's tomb, south of andorhal, standing on a tree stump. Map of the general area available at https://www.wowhead.com/tbc/index.cgi?m=12670 .

Cool Myranda the Hag will give the quest, Oculus Illusions.

Oculus Illusions
Travel to Blackrock Spire and collect 20 Black Dragonspawn Eyes. Return to Myranda the Hag when the task is complete.

9) Once again, you will have to raid uBRS and collect the eyes off dragonkin inside. They drop 1-2 at a time and are common.

10) Return to Myranda the Hag with 20 Black Dragonspawn Eyes. She will give you an Amulet of Draconic Subversion and the quest, Emberstrife.

Emberstrife
Travel to the Wyrmbog in Dustwallow Marsh and seek out Emberstrife's Den. Once inside, wear the Amulet of Draconic Subversion and speak with Emberstrife.

11) Travel to Emberstrife's Den in Dustwallow Marsh. It is the cave along the cliffs to the very south. Map available at https://www.wowhead.com/tbc/index.cgi?m=214014 . The Amulet of Draconic Subversion will transform you into a Black Scalebane once inside Emberstrife's Den. While disguised, speak to Emberstrife. He will behave as an NPC.

12) Emberstrife will begin a mini-series of quests titled The Test of Skulls. The first such quest in this series is The Test of Skulls, Scryer.

The Test of Skulls, Scryer
You must find the blue dragonflight drake champion, Scryer, and slay him. Pry his skull from his corpse and return it to Emberstrife.
You know that Scryer can be found in Winterspring.
The Skull of Scryer: 0/1

13) Travel to Winterspring with a good group of five* and enter the caves of Mazthoril, south of Everlook. Work your way in, and slay Scryer.

* Note: The drake kill quests may NOT be raided!

14) Return to Emberstrife using the Amulet once again, and get the followup, The Test of Skulls, Somnus.

Destroy the drake champion of the Green Flight, Somnus. Take his skull and return it to Emberstrife.
The Skull of Somnus: 0/1

15) Somnus can be found in Swamp of Sorrows, wandering in an arch up and down the zone between the Pool of Tears and the beach.

16) Return to Emberstrife as above, for the followup, The Test of Skulls, Chronalis.

The Test of Skulls, Chronalis
Guarding the Caverns of Time in the Tanaris Desert is Chronalis, child of Nozdormu. Destroy him and return his skull to Emberstrife.

17) Travel to Tanaris. Along the east edge of the zone (visible on map) is a mountain. Enter the area to find two low level drakes, and finally, Chronalis, guarding the door in the back.

The Test of Skulls, Axtroz
Travel to Grim Batol and track down Axtroz, drake champion of the Red Flight. Destroy him and take his skull. Return the skull to Emberstrife.

19) Go to the Wetlands and enter the Dragonmaw Gates region, the curved valley area in the east side of the zone. Travel down the valley a ways and you will find Axtroz.

20) Return to Emberstrife to receive the followup, Ascension...

Ascension...
It would appear as if the charade is over. You know that the Amulet of Draconic Subversion that Myranda the Hag created for you will not function inside Blackrock Spire. Perhaps you should find Rexxar and explain your predicament. Show him the Dull Drakefire Amulet. Hopefully he will know what to do next.

21) This quest is to simply find Rexxar again. Rexxar will give you the final quest, Blood of the Black Dragon Champion.

Blood of the Black Dragon Champion
Travel to Blackrock Spire and slay General Drakkisath. Gather his blood and return it to Rexxar.
Blood of the Black Dragon Champion: 0/1

NOTE: Only one person can get the blood off General Drakkisath per raid! For this reason, please organize a list of people who need it, or simply roll each time until everyone is complete.

22) Return the Blood of the Black Dragon Champion to Rexxar, and receive the Drakefire Amulet, a necklace, stats available at https://www.wowhead.com/tbc/item=4829/dreamers-belt .

23) Keep the Drakefire Amulet in your inventory to be allowed access into Onyxia's Lair when we raid! You need not WEAR the amulet, simply have it on your character.

CONGRATULATIONS! You are now eligible to raid Onyxia, Brood Mother of the Black Dragonflight!

To answer the question; " Does everyone in raid group need the drakefire amphulet need it?"

Answer is Yes you do ALL need it. Its like being attuned to the core. You can get in raid but unless youve been attuned to teh core, you just jump into the lavae instead of core. So, If you gte a pickup group together for Onxyia's Lair, make sure everyone has this amphulet or they wont be able to get inside her area. It serves not only as necklace, but a key as well.

Just as it is stated below, I would say this is the best abbreviated version I have found so far.

Alliance quest chain:
1) Dragonkin Menace (Elite) from Halrendis in Morgan's
Vigil, Burning Steppes.
2) Talk to Marshal Maxwell in Morgan's Vigil.
3) Talk to Magistrate Solomon in Lakeshire.
4) Talk to Highlord Bolvar Fordragon in Stormwind.
5) Talk to Magistrate Solomon in Lakeshire.
6) Talk to Marshal Maxwell in Morgan's Vigil.
7) Find Marshal Windsor in Blackrock Depths jail.
8) Return to Maxwell.
9) Find a random "A Crumpled Up Note" drop from a
random dwarf in Blackrock Depths.
10) Take said note to Marshal Windsor.
11) Retrieve Marshal Windsor's Lost Information from
General Angerforge and Golem Lord Argelmach (two
pieces).
12) Escort Marshal Windsor out of Blackrock Depths
jail.
13) Meet Marshal Windsor at gates of Stormwind.
14) Escort Marshal Windsor to Stormwind Keep to unveil
who Lady Katrana Prestor really is. (Spoiler: Onyxia)
15) Stand way the hell back while Bolvar fights the
elite dragons.
16) Travel to Winterspring to find Haleh. Find lots of
elite blue dragons and run to the back of the cave,
where you'll see a rune on the floor. It's hard to
see, so you will die a few times.
17) Haleh tells you to go kill Drakkisath in Upper
Blackrock Spire. You will need 15 fairly skilled
players for this quest, including yourself.
18) Once you have the "Blood of the Black Dragon
Champion", return to Haleh. Drakkisath drops only ONE
of these per kill. Yes, one for the entire 15 man
group. Not one per person, one per group.
19) Return to Haleh with the Blood and claim your
Drakefire Amulet.

Drakk drops 2-4 blood each time now. You can only see it if you have the quest. Not lootable if you don't.

You can still mountain walk to Halah in WS after the 1.9.3 patch. You will need to mount up and jump across the ravine and then jump again to get up to her. Confirmed myself, you can use a regular or an epic mount. It's NOT simple to do, so you may have to try 50 times until you get the hang of it.

Just be careful you don't get near the blue teleporter when you run up to her, as you may find yourself teleported on top of a dragon and dead before you even zone in below.

I have done the Onyxia pre-q both on the horde and alliance, horde's quest is longer, but alliance is harder

I did this last nite from the first visit to UBRS, all the way through.

Even though every step explains itself, thank you to 'TheSkeeve' (above) for a very accurate run down =)

And for what its worth, 4 Bloods dropped from Drakki at the end. Only I needed it!

all you need is a warlock and a hunter.
hunter pulls drakki to beast, feign death.
while the lock fearspams one guard and the other one is killed, then kill other guard. if both aint dead before hunter feigns, then let drakki come back a bit and kite him again

I think a maximum of 4 bloods drop, but I do know that only as many bloods will drop as people need them. Thus, the 4 dropped and 4 needed them =)

From the new patch everyone in the raid will get it and its only a 10 man raid now drakk is a hard boss to do with those guards with him.but if you kill him everyone in raid will get the blood if they have that qeust.

...while cleaning out your bank to make room for BC items? Now you need it to help noobs. Don't worry! Don't open a ticket with a GM. You can get another one by speaking with Champion of the Horde Rokaro in Shadowprey Villiage. He is Rexxar's brother, Rexxar is now in Outland. Rokaro (25.70) will give you another amulet.

You need to be at least lvl 50, be in a raid group and have the Drakefire Amulet (either in your inventory or equipped) to enter Onyxias Lair. This goes for all in the raidgroup. You will find the Lair in Dustwallow Marsh at 53.77.

Long quest chain but if they nerf it then people who spent ages on it will complain. i think they should at least make onyxia drop 70 items now though =/

i might sell my horde char and make an ally guy just to do onyxia. The horde quest chain is so much longer and so much harder. All the alliance have to do is walk around and talk to people.

wow I just noticed this, the horde version of the questline is like a billion times harder to get than the alliance version. They have to kill dragons and we have to rescue someone from prison! The horde's is harder, but alot more interesting.

You might not see some ! if your level is too high because some quests can already be started under level 50. So you might need to talk to the quest givers directly to see if they offer a quest.
